Development of Neuronavigator Utilizing Diffusion-Tensor Imaging

Abstract
Basal ganglia is the most frequent site of intracerebral hemorrhage. In this type of hemorrhage, injury of the internal capsule, which contains sensory-motor nerve fibers, is the most important prognostic factor. During surgery, avoiding injury to these nerve fibers, as well as blood vessels, is crucial to minimize post-operative neurological deficit. In this study we have developed a program for the image guided neurosurgery for cerebrovascular disease around eloquent nerve fibers, such as the internal capsule at the basal ganglia. For this purpose, we utilized diffusion-tensor imaging (DTI), which can visualize nerve fibers in the white matter, and displayed those nerve fibers on the screen of the neuronavigator in addition to the blood vessels, so that the neurosurgeons can use this program during cerebrovascular surgery to avoid injury to the important nerve fibers as well as blood vessels.
